Roster Maintenance Form Uses and Application


Roster Certification Request Form

  • Submitted to:Assistant Director of Compliance
  • Submitted from: Head Coach
  • Submitted when: By July 1 of each year
  • Purpose: To ensure athletic eligibility certification for all individuals who will be participating in countable athletically-related activities in the coming year.
  • Other considerations: This form is the PRIMARY trigger for the “coding” of student-athletes in the campus academic database, and also leads to the generation of accurate and complete squad lists.

Tryout Approval Form

  • Submitted to: Director of Compliance
  • Submitted from: Student-athlete wishing to Tryout
  • Submitted when: Student-athlete and coach agree to a 14 day tryout period for the student-athlete with the team
  • Purpose: To ensure compliance with all NCAA regulations regarding participation in athletically-related activities prior to the beginning of a 14-day tryout window
  • Other Considerations: This form simply “kicks off” a student-athlete’s tryout period. The head coach must still submit a Change of Status Form before the conclusion of the 14-day tryout window to maintain the student-athlete’s participation with the team beyond the tryout period and add them to the team’s squad list.

Change of Status Form

  • Submitted to: Assistant Director of Compliance
  • Submitted from: Head Coach
  • Submitted when: A student-athlete is to be added or deleted from the team during the academic year
  • Purpose: To formalize and communicate addition and deletion of student-athletes from the team roster with the Office of Athletic Compliance
  • Other considerations
    1. Compliance should receive the form IMMEDIATELY upon the addition or deletion of a student-athlete to avoid participation violations.
    2. Once a student-athlete has been deleted, they are INELIGIBLE to participate until they are re-added using this form.

Male Practice Player Form

  • Submitted to: Director of Compliance
  • Submitted from: Coach wishing to add male practice player to their women's team
  • Submitted when: Student-athlete has completed NCAA paperwork with compliance and student has received a physical from Athletic Training
  • Purpose: To ensure compliance with all NCAA regulations regarding male practice player eligibility
  • Other Consideration: This form is the final piece in the request by a women's team to add a male practice player. The compliance paperwork and athletic training physical MUST be completed before submitted the form. Once this has occurred and the form submitted, the Office of Athletic Compliance will review the request and notify the coaching staff via e-mail and updated squad list of the student-athlete's eligibilty for participation as a male practice player.

Annual Participation Form

  • Submitted to: Assistant Director of Compliance
  • Submitted from: Head Coach
  • Submitted when: Any time following completion of the team’s competitive season. Not later than June 1 for all teams
  • Purpose: To record the participation (compete, non-compete, medical, etc.) status of all student-athletes rostered with the team in the last academic year.
  • Other Considerations: Accurate documentation on this form is essential to maintaining accurate records on the NCAA database. Such documentation is essential for pursuing waivers of eligibility, providing accurate information to the media, and ensuring teams are not competing with players who have exhausted eligibility.

End of Year Delete Form

  • Submitted to: Assistant Director of Compliance
  • Submitted from: Head Coach
  • Submitted when: Any time following the end of the academic year. Not later than June 1 for all teams
  • Purpose: To record the reasons an individual parted ways with the RMU team that rostered them in the previous year.
  • Other Considerations: Accurate documentation on this form is essential to “closing” the files of student-athletes that required monitoring in the previous year. Such documentation is critical for report submission to the NCAA and other entities seeking information on student-athlete retention.
