STUDENT-ATHLETE REPORTING FORMS


A major responsibility of Compliance is to monitor various types of information and activities. Monitoring allows Compliance to gather and review information in a proactive manner. Student-athletes are required to complete both NCAA and UAA compliance forms. These forms are filled out at the beginning and end of each academic year and, in some cases, at mid-year. Each student-athlete’s forms are kept in an individual file folder in Compliance.

The forms at the beginning of the year must be completed before student-athletes are eligible to practice. Compliance will meet with the sports of cross-country, football, soccer, and volleyball before the first day of classes in the fall, and the staff will meet with the other sports during the first week of classes.

VEHICLE REGISTRATION INFORMATION

  • On the vehicle registration form, student-athletes are required to provide a description of the vehicle, license tag information, and provide information regarding the titleholder and the purchase of the automobile.
  • Compliance reviews and approves the completed vehicle registration forms.
  • If concerns are raised, they are brought to the attention of the AD.
  • The course of an investigation will vary according to the circumstances; however, generally the first step is to review license tag information with the University Police Department.
  • Compliance may request additional information and conduct an inquiry if concerns remain.

STUDENT-ATHLETE INFORMATION

  • Student-athletes indicate their local address and telephone number, as well as their parents’ home address and telephone number.
  • Student-athletes also indicate if they have ever attended another institution, and if so, did they participate in intercollegiate athletics.
  • This form also requires student-athletes to indicate whether or not they signed an NLI at the previous four-year institution.

AMATEURISM AND EXTRA BENEFITS

On the amateurism and extra benefits form, student-athletes are required to provide information on a number of areas including sports agents, professional contracts, complimentary admissions, gambling, and autographs.

STUDENT-ATHLETE CONFIDENTIALITY AGREEMENT

On the student-athlete confidentiality agreement form, student-athletes agree neither to discuss nor release any secure or sensitive athletic department information to parties not designated for this information by senior university administrators. This information includes:

  • Academic records
  • NCAA, NEC or RMU investigations
  • Unproven or unsubstantiated allegations regarding student-athletes or university administrators

ETHICAL CONDUCT AGREEMENT

On the ethical conduct agreement form, student-athletes affirm their understanding of NCAA, NEC and RMU rules, in addition to the NCAA’s ethical conduct requirements as portrayed in bylaw 10.1.

SPORTS GAMBLING AGREEMENT

Student-athletes are required to read and affirm their understanding of multiple examples of sports wagering and its deleterious impact on their eligibility. They then sign the document agreeing to abide by all NCAA rules regarding these topics.

NORTHEAST CONFERENCE SPORTSMANSHIP POLICY

Student-Athletes are required to read and sign the NEC’s principles and policies on sportsmanship, including examples of poor sportsmanship warranting punitive measures. With their signature, student-athletes agree to conduct themselves within the standards of the NEC and RMU.

SUMMER EMPLOYMENT

  • On the employment form, student-athletes are required to provide information about employment during the past summer.
  • Information on the student-athletes employer is also required. The student-athlete is required to list the dates of employment, as well as information regarding the amount of money earned during the summer.
  • Student-athletes are also required to report whether or not they plan to work during the academic year.

FINANCIAL INFORMATION

  • On this form, student-athletes are required to report their intentions to receive a Pell Grant, any outside scholarships, or loans.
  • Student-athletes also are required to report whether or not they, or a member of their family, has received any benefits from a RMU booster.

NCAA DRUG TESTING CONSENT

  • Student-athletes are required to sign the NCAA drug testing consent form. This form explains the drug testing program, as well as requests the consent of the student-athlete to be tested.
  • If a student-athlete has not reached his or her eighteenth birthday, the drug testing consent form is sent to the parents or legal guardian(s) for their signature and approval.

NCAA STUDENT-ATHLETE STATEMENT

  • Student-athletes are required to read and complete the Student-Athlete Statement that affirms that the student-athlete has been provided a copy of the Summary of NCAA Regulations; meets NCAA regulations regarding eligibility, recruiting, financial aid, amateur status, and involvement in organized gambling; and all information provided to the NCAA, the Eligibility Center, and the institution’s admissions office is accurate and valid.
  • Student-athletes also read and complete the second part of the Student-Athlete Statement that includes the Buckley Amendment. By signing this form, student-athletes consent to disclose pertinent information only to the authorized university representatives. Disclosing records is used only when determining eligibility for intercollegiate athletics.
  • Student-athletes also read and complete the third part of the Student-Athlete Statement that affirms that they have not violated any amateurism rules and have not provided false or misleading information concerning their amateur status to the NCAA, the Eligibility Center, and the institution’s athletics department, including administrative personnel and the coaching staff.
  • Student-athletes also read and complete the fourth part of the Student-Athlete Statement that authorizes the NCAA (or a third parting acting on behalf of the NCAA to use their name or picture to generally promote NCAA championships or other NCAA events, activities, or programs.
  • Student-athletes also read and complete the fifth part of the Student-Athlete Statement that affirms that they have not tested positive for banned substance by a non-NCAA athletics organization.
  • Incoming transfer student-athletes also read and complete the sixth part of the Student-Athlete Statement that affirms previous involvement in NCAA rules violation at any previously attended institutions.
  • Finally, incoming freshman student-athletes complete the seventh part of the Student-Athlete Statement that affirms that they have received a validated SAT and/or ACT score.

HIPAA

Student-athletes are required to read and sign the NCAA HIPAA form which grants RMU permission to disclose to the NCAA health information regarding injury, illness, or participation related to participation.

RELEASE OF INFORMATION CONSENT

Student-athletes are required to read and sign the RMU Release of Information Consent Form. By signing this form, student-athletes give various departments within RMU permission to provide and receive need-to-know information related to student-athletes’ academic, athletic and medical records.

INTERNATIONAL STUDENT-ATHLETES

International student-athletes are required to sign the General Amateurism and Eligibility Form for International student-athletes that affirm that these students meet all NCAA regulations regarding eligibility, recruiting, financial aid, and amateur status.
